Self-referral disclosure. Daniel Seidler built the STAN assessment and also provides physical therapy through Gowanus PT. Gowanus PT is shown on the result screen as a separate option above the matched clinic list. It is not one of the 15 listed clinics, is not scored or ranked against them, and appearing there confers no ranking advantage on it. Gowanus PT has no clinic location — care is delivered by video anywhere in New York State, or in person at a patient's home or gym in Brooklyn. Users who indicate they intend to use insurance are shown, at the top of that block and before any description of the service, that Gowanus PT bills no insurance and participates in no network.
Treatment without a referral. Under New York Education Law §6731, a licensed physical therapist with at least three years of full-time practice may evaluate and treat a patient without a referral for up to 10 visits or 30 days, whichever occurs first. Daniel Seidler has practiced for 25 years and meets that threshold. As the statute requires, patients are advised in writing before treatment begins that a health plan or insurer may not cover treatment provided without a referral. An evaluation alone does not count toward the 10 visits unless treatment is provided at the same visit.
Payment and insurance. Gowanus PT does not process insurance claims and participates in no insurance network. The initial 15-minute consult is free. Thereafter the rate is $250 per visit, or $2,250 for a ten-visit package, with payment due before or at the time of treatment. Patients receive an itemized statement for each visit, including CPT and ICD-10 codes, which they may submit to their insurer for reimbursement according to their own coverage. Reimbursement is a matter between the patient and their plan and is not guaranteed; many plans limit or exclude out-of-network physical therapy, and some apply different terms to telehealth. Diagnosis codes on a statement are derived from the physical therapist's own evaluation, not from the STAN classification, which is an educational assessment and not a diagnosis.
Medicare. Physical therapists are not included in the federal opt-out provisions available to physicians and certain other practitioners, and Medicare's mandatory claims submission rule governs how covered services provided to beneficiaries must be billed. Gowanus PT therefore cannot accept direct payment from a Medicare beneficiary for a Medicare-covered service in the ordinary course, and Medicare beneficiaries are asked to make contact directly rather than book through the standard scheduling flow.
Telehealth. Physical therapists are authorized telehealth providers in New York. Before a video visit, identity is verified, informed consent is obtained and documented, and the patient's physical location is recorded so that emergency services can be directed correctly. A physical therapy license follows the patient's location, so video visits are available only to patients physically present in New York State. New York workers' compensation regulations do not permit physical therapy by telehealth; work-injury cases are handled in person or referred.
